Abstract
ABSTRAK Ketidaknyamanan yang sering terjadi pada ibu hamil trimester III adalah sering kencing. Apabila keluhan sering kencing ini tidak teratasi bisa mengarah ke Infeksi Saluran Kemih (ISK). Maka dari itu, dilakukan asuhan yang komprehensif. Hasil studi pendahuluan yang dilakukan di TPMB “MY” terdapat 7 orang dari 19 ibu hamil UK ≥ 36 minggu. Jenis penelitian yang digunakan adalah penelitian deskriptif dengan pendekatan studi kasus. Dilaksanakan di TPMB “MY” tanggal 17 April – 30 April 2025. Pada kasus ini yang menjadi subjek adalah perempuan “AM” G2P1A0 UK 38 Minggu 1 Hari dengan masalah sering kencing yang dilakukan di TPMB “MY” yang beralamat di Desa Tegallinggah, Kecamatan Sukasada. Metode pengumpulan data yang digunakan yaitu wawancara, observasi, pemeriksaan fisik dan studi dokumentasi. Pengumpulan data menggunakan instrumen sebagai acuan dalam memberikan asuhan. Hasil asuhan pada ada pertemuan pertama ibu mengalami keluhan sering kencing dimalam hari yang mengganggu waktu tidur, diberikan cara mengurangi sering kencing dan senam kegel. Pertemuan kedua sering kencing ibu dimalam hari sudah teratasi. Pada masa persalinan, kala I berlangsung 8 jam 10 menit, kala II 20 menit, kala III 7 menit dan kala IV 2 jam tanpa penyulit. Bayi lahir tangis kuat, gerak aktif, jenis kelamin laki-laki. Pada asuhan kala II terdapat kesenjangan seperti asuhan persalinan tidak dilakukan secara keseluruhan yaitu diantaranya tidak melakukan pemeriksaan lilitan tali pusat, tidak menunggu putar paksi luar dan tidak melakukan sangga susur. Pada asuhan bayi baru lahir terdapat kesenjangan yaitu tidak melakukan IMD dan tidak melakukan pemeriksaan fisik lengkap pada bayi baru lahir. Pada masa nifas ibu tidak ada komplikasi dan penyulit. Tidak melakukan kunjungan KF II dan KF III serta KN II dan KN III karena da kesibukan, diharapkan kedepannya tenaga kesehatan memantau pasien dan dapat mengunjungi pasien kerumah jika pasien kesulitan datang kelokasi atau TPMB. Disarankan kepada bidan dapat meningkatkan asuhan secara komprehensif dalam melakukan pelayanan yang berkualitas bagi kesehatan ibu, anak dan masyarakat. Kata Kunci : Ibu Hamil Trimester III, Sering Kencing, Senam Kegel, Continuity of Care (COC). ABSTRACT A common discomfort experienced by third-trimester pregnant women is frequent urination. If left unaddressed, this condition can lead to Urinary Tract Infections (UTIs). Therefore, comprehensive care is essential. A preliminary study conducted at the Independent Midwifery Practice (TPMB) "MY" revealed that 7 out of 19 pregnant women with a gestational age (GA) of $\ge$ 36 weeks faced this issue. This study employed a descriptive method with a case study approach, conducted at TPMB "MY" from April 17 to April 30, 2025. The subject was Mrs. "AM", a G2P1A0 woman at 38 weeks and 1 day of gestation, who presented with frequent urination at TPMB "MY", located in Tegallinggah Village, Sukasada District. Data were collected through interviews, observations, physical examinations, and documentation reviews, utilizing standardized instruments as a guideline for care. The results showed that during the initial visit, the patient complained of nocturnal frequent urination that disrupted her sleep; she was subsequently taught management techniques and Kegel exercises. By the second visit, the issue was successfully resolved. During delivery, the first stage lasted 8 hours 10 minutes, the second stage 20 minutes, the third stage 7 minutes, and the fourth stage 2 hours, all without complications. The baby was born male, crying vigorously, and actively moving. However, discrepancies were noted during the second stage of labor care, as standard delivery procedures were not fully implemented, such as omitting the check for umbilical cord entanglement, failing to wait for external rotation, and not performing the support-and-slide (sangga susur) technique. Discrepancies were also found in newborn care, specifically the absence of Early Initiation of Breastfeeding (EIBF) and the omission of a complete newborn physical examination. During the postpartum period, the mother experienced no complications. However, the second and third postpartum visits (KF II & KF III) as well as newborn visits (KN II & KN III) were missed due to the patient's personal schedule. Moving forward, healthcare providers are expected to monitor patients more closely and conduct home visits if patients encounter difficulties traveling to the clinic. Furthermore, midwives are advised to optimize comprehensive care to ensure high-quality services for maternal, neonatal, and community health.
